RECEIPT STYLE SELECTION

A short form to tell us how you would like to receive your Therapy session receipts. Our system default is to send receipts by email with the receipt information in the body of the email. If this is fine for you you don't need to do anything.

If you prefer to receive receipts as a PDF document attached to an email please simply check the box.

Please enter the unique Client ID to ensure your selection gets connected with the client record. Please use the format of clients first name followed by the parent mobile number with no gaps. For example - Sarah0400444000
